SHORT QUOTATION CALL NOTICE
OSACS invites sealed quotation from authorized distributors/ dealers having
valid VAT registration & Drug Licence for supply of Cotrimoxazole –DS tablets for
HIV TB programme. The detailed terms and conditions are available in the
website www.orissa.gov.in and www.osacs.nic.in, which can be downloaded for
use. Interested parties may submit their quotation super-scribing as “Quotation
for Cotrimoxazole –DS Tablets” to the undersigned through speed post /
Registered Post / courier services/ Drop Box only which should reach on or
before 4.00 P.M of 22nd November, 2013 positively.

1.) Basic regimen is “Cotrimoxazole –DS (Trimethoprim IP 160 mg. + 2.) The Quotationer shal quote the rate per strip/bottle, which wil be valid up 3.) The Quotationer shal indicate the quantity in each strip/bottle 4.) The taxes wil be written separately. 5.) Price format should be signed by the quotationer with seal. 6.) The selected firm wil supply the tablets as & when required by OSACS within 30 days from the date receipt of the conformed supply order. 7.) The quotationer shal submit the up to date VAT Clearance Certificate. 8.) The quotationer shal submit the copy of valid drug license. 9.) Quotation shal accompany with the copy of GMP certificate of the 10.) The Quotationer should submit the copy of the manufacturer’s 11.) Any delay in delivery shal render the supplier liable to be charged liquidated damages @ 0.5% per week or part thereof subject to maximum of 10% of order value, which wil be deducted from the payment due. 12.) Delivery should be made on OSACS’s Head Quarter, BBSR. 13.) Ful payment wil be made after completion of delivery as per specification and subject to production of invoice/bil against order. 14.)
